ESOLUTION NO. 15 Q 3
A RESOLUTION AUTHORIZING THE CITY MANAGER TO
EXECUTE A PROPOSAL -BINDER AND OTHER NECESSARY
DOCUMENTS, WITH SWINFORD-HOFFMAN & ASSOCIATES,
CENTURY PLANNERS, AND PERICO FOR EMPLOYEE
HEALTH INSURANCE
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF CAPE GIRARDEAU,
MISSOURI, AS FOLLOWS:
ARTICLE 1. The City Manager, for and on behalf of the City of
Cape Girardeau, Missouri, is hereby authorized to execute a
Proposal -Binder and other necessary documents, with Swinford-
Hoffman & Associates for broker -agent services, Century Planners
for Third Party Administrators, and PERICO for re -insurance. A copy
of said Proposal -Binder is attached to this Resolution and made a
part hereof.

Dear Ken:
Based on Dan Ward's letter of November 8, 199.9, Perico. has revised their
quotation slightly. I am pleased to advise you that Perico will agree.to remove
the original quote contingencies in return for several action dates and a premium
deposit. Please refer to the cover sheet of the Perico Excess.Loss insurance
Proposal,
You will note that the rates remain very close to the original quotation (some
factors are lower). The slight modifications were due to an adjustment for the
assumed vs. actual police and fire population. Further there is a modest
difference depending on the network the City will choose.
We are pleased that we are able. to now present this as a firm offer, subject to
Perico's requirements_
