Home My WebLink AboutRes.2921.10-05-2015BILL NO 15-183 RESOLUTION NO A RESOLUTION AUTHORIZING THE CITY MANAGER TO EXECUTE A SUPPLEMENTAL AGREEMENT NO 2 WITH KOEHLER ENGINEERING AND LAND SURVEYING, INC , FOR THE INDEPENDENCE AND GORDONVILLE INTERSECTION DESIGN BE IT RESOLVED BY THE COUNCIL OF THE CITY OF CAPE GIRARDEAU, MISSOURI, AS FOLLOWS ARTICLE 1 The City Manager, for and on behalf of the City of Cape Girardeau, Missouri, is hereby authorized to execute a Supplemental Agreement No 2 with Koehler Engineering and Land Surveying, Inc , for the Independence and Gordonville intersection design, in the City of Cape Girardeau The Agreement shall be in substantially the form attached hereto as Exhibit A, which document is hereby approved by the City Council, and incorporated herein by reference, with such changes therein as shall be approved by the officers of the City executing the same PASSED AND ADOPTED THIS ' DAY OF ATTEST Gayle U Conrad, City Clerk al -I Harry E edig-er, Mayor SUPPLEMENTAL AGREEMENT NO. 1 TO ENGINEERING SERVICES CONTRACT This Supplemental Agreement is made part of an agreement dated October 10, 2014 between the City of Cape Girardeau and Koehler Engineering and Land Surveying, Inc. for traffic study and conceptual report of project STP-1500(022). The purpose of this Supplemental Agreement is to include engineering services for design and construction and prepare PS&E plans for the intersection improvement concept (a single-lane roundabout) identified per the study performed under the original ESC. The overall services provided for this project shall be in an amount not to exceed (Fifty Seven Thousand, Eight Hundred Fifty-Five Dollars and 44 Cents) ($57,855.44) without further authorization. The total design phase services shall be in an amount not to exceed (Forty Thousand, Seven Hundred Eighty-One Dollars and 83 Cents) ($40,781.83). The total construction phase services shall be in an amount not to exceed (Eight Thousand Three Hundred Thirty Four Dollars and 41 Cents) ($8,334.41). Attachment A outlines the cost breakdown for this Supplemental Agreement. Anticipated Dates for Milestones shall be established as follows: NTP with Preliminary Engineering: March 2,2015 Submission for Environmental/Historical Clearances: March 16, 2015 Completed Preliminary Design Tasks as noted in Attachment'A': April 30,2015 Right of Way Plan Submission May 7, 2015 Completed Final Design Tasks as noted in Attachment 'A': June 19, 2015 Receipt of City/MoDot Comments: July 3, 2015 Address Comments and Final Submittal of PS&E: July 17, 2015 Advertise for Bidding: August 2,2015 Supplement Agreement No. I accepted as defined herein: FIG. VI - 4 Revised 02-14-00 ATTACHMENT A SUPPLEMENTAL AGREEMENT NO. 1 (October 10,2014) DESIGN Phase Services: 1) Survey and research existing right of way, easements and property lines 2) Survey and research existing and proposed utilities 3) Perform 3D and topographic survey of the intersection site including trees, signs, buildings and other adjacent features 4) Prepare historic/heritage/environmental submittals 5) Prepare exhibit for presentation purposes 6) Attend meetings with property owners as needed 7) Prepare preliminary design of a single-lane, offset-left roundabout including: a. horizontal geometric design elements including curb and gutter/lane edge lines, truck apron(s), splitter islands, pedestrian crossing(s) b. profiles of circulation lane and approaches c. existing and proposed entrances/driveways which fall within the limits of improvement. d. existing and proposed utility locations e. storm water drainage features(if necessary) f. interim and presumed future accommodations for pedestrians g. typical sections h. exhibits and descriptions for easements i. documentation of performance checks including: fastest path analysis, AutoTum for WB-65, sight distance, level of service and v/c ratio for design year traffic j. opinion of construction cost 8) Prepare final design of the intersection including the following: a. horizontal and vertical alignment of the street approaches with coordinates b. storm water drainage design (if necessary) c. design of interim accommodations for pedestrians d. water main relocation design e. street lighting design f. typical sections of street and roundabout g. pavement elevation detail sheet with spot elevations 10' to 20' apart along inside and outside edges of all lanes h. job special provisions i. traffic control plan with proposed staging j. permanent striping/signing plan k. retaining wall design (if necessary) I.
